Amazing place! The staff is awesome, all do their work with care!
Very hard to find parking the road in and out is super bumpy and for sick people they could trip and hurt themselves. The nurses seemed good who were caring for my friend
My only experience at a skilled nursing facility twice has been OUTSTANDING, from the top Administrator Curtis ( open door policy)to the lead in the front Helen whom is like a big sister and looks out for you 100%!! Now the meat & potatoes: PT room is full of OUTSTANDING rock stars that WILL GET YOU BACK TO ” normal “, last and CERTAINLY NOT LEAST: the kitchen, fugetabahboutit Michael, Atik& My hero Madeline not to mention all the other staff that looks out for you!!! I 100% recommend this place & the nursing staff and house cleaning deserve honorable mention. Very happy that I was fortunate enough to be place there and met great people that I can consider friends after my departure!!!!
My mother’s was admitted in Hayward Garden last 11/2025, recently discharge on 2/11/2026. The staffs, CNA’s, all the Nurse’s, Supervisor’s, Pt’s, PTA’s, OT’s not to mention kitchen staff. They are all knowledgable of their responsibility. Helped my Mom to get stronger, and recover from surgery. She needs every help she can get. She is 90 years old. Everyone was patient and kind. They all be been very helpful and accommodating to my mother’s needs. When ever I have a question or my mother needs anything all I have to do is communicate it with the staff and it’s handled and take cared of right away. Upon discharge or any request I have I communicate it directly to the Social worker Director, Maria. She has been extremely helpful and great in communicating any matter regarding my mother and any documents needed. All in all I am satisfied in having my mother stay at Hayward Garden. Mother is also happy with Hayward Garden’s service.
Our dad was admitted for both wound care and PT, with a very challenging situation. During his stay, we got to meet so many wonderful staff whose kindness, gentleness, caring, professionalism, and energy made us feel that he was in a good place, which helped to dispel our concerns regarding leaving him in a skilled nursing facility. Sadly, he wasn’t well enough to be able to take full advantage of all the wonderful care given. Our special recognition to Samir, Kshitij, Shafali, Amoljeet, Paola, Stephanie, Tin, Patrick, Ashmita and Teresa. Teresa – your smiles and energy are such a blessing here. Thank you very much for so quickly arranging to get the special mattress for our dad. Grateful to the kitchen staff or all the changes they made for his meals. Also, thanks so much to John for quickly taking care of an important situation for us with Kaiser. Sadly, our dad transitioned to hospice care here on the 12th day of his stay. He needed periodic, routine check-ins to ensure his comfort, especially as he quickly became more ill and less responsive. While we really liked everyone here and they’re very nice (special kudos to CNAs Paola, Stephanie & Tin, wound care nurse Samir, and Michael in the kitchen), twice several important well-checks relating to IV fluids were missed that resulted in severe fluid retention and unnecessary pain for my dad. Also, he was about to be weighed the day before he passed and I’m so glad I was there to stop the procedure. In hindsight I wish that his PT and OT sessions could have been assessed and adjusted to meet his condition, especially as his nutritional intake continued to decline, rather than expecting him to perform activities at the level that are perhaps typical for many patients in rehabilitation; I suppose it’s challenging for all dimensions of care to get the whole, changing picture when it’s downward as opposed to upward. It’s with a sad heart that I’ve had to update this review, really wanting to say that the entire experience was great. Hopefully this is a learning lesson for all. Nevertheless, I was impressed by the regular cleaning being done, checking of smoke detectors, the daily activities. The vibe to us was all-around nothing less than positive. Curtis, when the patients’ rooms are finally done as you mentioned in another post, HGPA will truly be a gem all-around. Keep up the great work, everyone! You are all very important and appreciated.

THERAPY IS AMAZING ALL OF THE THERAPIST ARE AMAZING, KIND AND REALLY LOVELE TO PACIENTS. Now when it comes to the nurses they sometimes seem to do not care my mom was there and I always found her with her pamper full when I asked for change they would come and seen upset( not sure why you go to school for this if you don’t enjoy it) anyhow also if your family member speak Spanish I do not recommend this place they do not have interpreters also they seem to have favoritism with some pacients as one I found my mom laying in bed with the food aside and the other paciente was already eating . Social worker always saids that she’s going to call you and she just never does .

First and foremost, I truly appreciated the warm and welcoming demeanor of your caregivers. Whether in person or over the phone, they consistently greeted us with smiles and were attentive to my mother’s needs. It was comforting to know that whenever I called, they would promptly take action, bringing my mother a warm cup of coffee or a cozy blanket when needed. Their dedication and compassion truly made a positive impact on her care. Additionally, the kitchen staff deserves special recognition. They were always friendly and approachable, ensuring my mother received her favorite foods, such as soft sugar cookies, coffee, sparkling water, and sandwiches. I was also impressed by the self-service ice box, which allowed me to easily get ice for my mother as needed. Moreover, I want to commend the overall cleanliness and organization of the facility. It provided a safe and pleasant environment, which further contributed to a positive experience for my mother. Thank you for the incredible support and care you provided. I am a surgeon at Washington Hospital in Fremont. I was supposed to operate on a patient at this facility and they dropped of this elderly patient at a wrong facility and left. They did not even check with the facility where they dropped off the patient if he belonged there. When I called and spoke to the supervisor Helen she was so nonchalant about it. I had a lengthy conversation with Mr. Porter who is the administrator and was informed that the mistake was with the transport company. The transport company was given the correct address for the drop off but they dropped off the patient at a different facility. It is unclear if the transport company was picked by the facility or by the patient’s insurance company. It took over two hours and numerous calls before another transport company brought him to the correct location. Once the patient did arrive at the hospital, we noticed that the patient was given blood thinners when explicit written and verbal instructions were given to stop blood thinners 5 days prior to surgery. So after all this ordeal the patient’s surgery had to cancelled and will be rescheduled for another day.

Nice rehabilitation facility… food is freshly made. The room was clean… and the help was great in terms of responding to my needs as a patient; in a timely manner. There was a minor pain medication hick-up; however, when I expressed my discomforts and charges …the highly astute manager , Brian, immediately resolved my plight. Moreover, Brian took the liberty to establish new system-wide business practices to assure the balance of my sojourn was elevated to first class.

The place was clean food wasn’t good at all my mom stayed their and she mostly enjoyed the pudding or me bringing her munchies. It was hard to get help or answers. The staff was very busy and underpaid for what they were doing some were very rude and impatient. Just a few were enjoying their job. The therapist was a joke. Didn’t do much just collecting a check. Wouldn’t recommend at all. Someone stold my mother’s reading glasses. 1 🌟

Great therapy team, Ru and Nina were great motivators. Alejandra was a thorough and caring speech therapist. Ashmita was great with treating wounds. Charles was always a perfect gentlemen serving her. And she loved seeing Vivian every morning when she weighed her. The staff was helpful. I would recommend Hayward Gardens!

Hayward Gardens proved to be a fine re-hab center for my sister. She appreciated the garden view, the light, airy environment, the therapists, and the amenities the activity director provided in the gathering room. We appreciated the efforts of the staff in making HG a positive experience.

My wife was here for four weeks. The care team was exceptional and worked hard to get her recovered. Some other parts of the operation have problems but they have an administration that is engaged and actively trying to improve things, so if you see something problematic, talk to them about it. You’ll find people wanting to help, and wanting to improve.

They almost killed my mother. She had two open wounds that was not being treated she ended up septic and had to be rushed to the emergency room. Whatever you do don’t put your love ones in their care.

After my brother was discharged from Eden Hospital, he was admitted to Hayward Gardens Rehabilitation Center. There, he shared a room with a patient who had pneumonia, which deeply concerned us. The patient himself informed us of his condition; we worried about my brother, who was suffering from heart failure, and we were especially anxious about the risk of him falling ill again. We immediately approached the front desk nurse to request a room change to protect his health, but we were told that no other rooms were available. And also, the nurse told us that the patient didn’t suffer from pneumonia. He was making it up. Unfortunately, a week later, my brother developed pneumonia and had to be transferred back to Eden Hospital. Despite medical efforts, my brother passed away a week later from severe pneumonia. This experience has profoundly shaken my faith in Hayward Gardens. No patient should have to endure such a situation, and it is heartbreaking to think that inadequate care and a lack of preventive measures could lead to such a devastating outcome. I strongly urge the manager of this place to reflect on these concerns and take immediate action to ensure that future patients do not face similar risks.

My mother was admitted recently and spent 2 weeks at Hayward Gardens. The location is nestled near a woody area and thus you will see many wild Turkeys which was the highlight of every day for my mom. The kitchen prepared food every day that was very good. The Nurses, CNA’s, PT’s, OT’s and supporting staff were very friendly and always open to any issues that arise. My mom would not have had quick recovery that she had were she not at Hayward Gardens. Special thanks to: Maria our social worker for helping throughout the process. Jane staff assistant for supporting our stay Nurses – Omar, Elizabeth, Margo for attenting to my mom’s needs CNAs Monica, Manpreet for being so kind and attending to my mom PT&OT Therapists – Cynthia, Rue, Patrick Thanks to all the many others as well that we just didn’t get their names.

Hayward gardens has very good and polite employees. They are very helpful when needed. Physical Therapy is excellent! The therapy staff is very helpful.
